5/1/2002 Interview with Roger Bloodworth Backround: Roger left early from work on 4/17/2002. He told me he was in pain. He indicated his abdomin and right side and right leg. He was out 4/18-4/19. During that time he came in and told me that he had a kidney infection. On Friday 4/19 I received a call from Soperton Pharmacy that he was there trying to get medication and charge it to Worker's Compensation. I told them I knew nothing of it. During the following week, I was travelling and Ron Hill questioned him about the call from the pharmacy. Roger said that he was sore from loading machines. He presented a verification form mentioning hurting his back 'loading the machines'. He also presented an ER discharge recommending 'ice to area for discomfort' and prescribing a pain killer. The note also said return if persists or worsens and to follow up with primary doctor. He also requested 2 days off, Thursday and Friday, for two weeks, so he could 'rest up'. I granted the request in lieu of a personal interview. Roger then requested that he take off Monday and Tuesday of the next week instead of Thursday and Friday so he could 'take care of some personal business'. The request was answered by saying that if he was okay he needed to come to work, if he was not okay, he needed to stay home. He came in for a moment on Monday and left almost immediately because his son 'might have to have surgery'. He called in that his son 'may have to have hernia surgery' again on Tuesday and did not come to work. On Wednesday, 5/1, he returned without any verification of his son's surgery. At that time we had an interview. He said he would get a verification from the doctor on his son's surgery. I asked him if he was injured on the job and he said yes. I asked him how he felt now and he said he was hurting. I asked him when it happened and he didn't know. He said 'the day I loaded #35'. I asked him why he didn't report it when it happened and he said he told Ron about it. Ron said and Roger agreed that all he said to Ron was that he was sore. I asked him why he told me it was a kidney infection and he said because he had blood in his urine and that's what he thought it was. I told him that as it stood, we would report a claim in the morning and that if he gave Worker's Comp the same vague information that he gave us, he would probably be declined. The doctor's note states that he needed to return if symptoms persist. I declined to allow him to work without a release from the doctor. I told him that when he did come back to work that he needed to also provide verification of his son's surgery. He asked me for an advance and I declined. He asked me for a written summary of the interview and I declined. CS
